Grow A Star Consent
An application to the Grow A Star youth support program has been received for <firstname> <lastname>.
In order for the application to proceed, parent or caregiver consent is required.
To enable us to assess whether a young person is eligible for the Grow A Star Program, we require some personal information. Personal information will be collected, stored and managed in accordance with our Privacy Policy.
